If I could post 0 stars I would. Arrive there then the bigger size woman at the front desk calls me — keep in mind I was waiting for 45 minutes patiently — to tell my my insurance isn’t active. I have great insurance so I leave there like after 45 minutes couldn’t tell me that she didn’t find my insurance I instantly call my insurance company –shocker they found me active– and I called them back to tell them that a 5 minute phone call actually trying to help out a client would’ve made all the different. Again the bigger lady at the front desk was ruin, inconsiderate and straight up lazy. Do not ever go there, literally go anywhere but this place.

I wouldn’t do your ADOT CDL test here. I am hiking the Grand Canyon on Thursday Rim to Rim, passed all the tests with flying colors of course and he only renews it for a year instead of the custom 2. I had a mitral valve repair without issue and that’s why. He won’t get my $ 100 again. Doctor Levy, you’re a nice guy and all, but you won’t get me back next year. Everyone was nice in the office, kinda dirty though.

This clinic has no female doctors. None, nada, nilch. Is that even legal? Doesn’t a female need to be present during certain procedures? Not even the receptionist is female.

I would give this place no stars if possible. Extremely slow, we waited over an hour to be seen. The office seemed disorganized. I would not recommend anyone to come here.

First visit for work. comp. for my husband’s severe back pain. The radiology tech was pleasant, but the physicians and intake admins freely discussed patients’ drug tests and other various results while laughing at how stupid they apparently were which is (obviously) super unprofessional and unethical. The doc who saw my husband had zero bedside manner, but hopefully his diagnosis and treatment are better than the clinic he’s employed at.
