Don’t do it. I’m an immigration attorney. Her exam submitted on behalf of my client was rejected. She used old expired forms. She did not stay up-​to-​date regarding DHS medical exam requirements. Then wanted the form from the attorney. She did not have the proper software or even an email address so I could send the link to the USCIS public access form. Unprofessional, outdated, waste of time.
She charged my client near $ 500. You can get efficient, more capable service elsewhere. Shop around, the doctors I recommend charge between $ 150 — 200. Never had an issue or rejection.
Don’t do it.

Please stay away from this doctor; she charged my friend over $ 500 for a medical exam required by Dept of Homeland Security in order to obtain his green card. DHS should remove her as a civil surgeon. My friend regrets going to her office and his attorney handling his immigration case indicated that the medical exam was too expensive. I cannot in good faith give her 1 star; but Unilocal does not allow me to not give her a star.
