I set up an appointment to get information about having sun spots removed from my face with laser treatments.
I showed up for my appointment and was told that they would not see me or give price information unless I gave them a copy of my driver’s license and filled out forms listing a great deal of personal information. This information included not only my driver’s license information, but my medical history and employment information. The forms also included a sheet stating that I sign off on a privacy policy (that was not attached) about how they can use my information.
I left without completing my appointment. There are far too many incidents of identity theft to be liberal with personal information. I will be taking my business elsewhere.
