This facility is OUTSTANDING! it was the very BEST places i could ever had my husband, a second time stroke patient at. Great, warm and caring CNA’s Nurses and Admin staff. Rehab Therapists too– SHOUT! If you come here and notice the modern, beautiful & homey rooms you’ll know its as close to home as close can be! They also sent visiting nurse to do medication mgmt–(thank you, Chantel) and a doctor and CNA for personal care too under hospice program. OVERALL, 5 star 5 star business! PROPS, and THANKYOU, GRACE!!! :) Job well done!!!

This location is now known as «Grace Healthcare of Pleasant Hill»( ). My father spent the last week of his life there under care from both the nurses on staff and Hospice of the East Bay. The staff was kind, caring, and competent. The facility itself is small and rather intimate; the rooms are not very updated. It is apparent that the staff cares greatly about their patients.

I cannot speak to the care delivered in this facility, only the process of trying to have a patient placed there. Starting in July 2013, we tried to place my mother, who is seriously debilitated following a stroke, in this facility. The facility is small and seems to have good care, but the main reason was proximity for my father, who could walk to visit her. We got all of my mother’s records to them, and they kept requesting additional information. The nursing director, Debbie, then left for vacation and gave me a contact name. When I called the contact, she had no idea and told me that I would need to talk to Debbie. Since her return from vacation three weeks ago, she has not returned my calls(I left two messages). This facility has the right to not accept any patient. However, they would not actually refuse, simply stringing us along over two months, and we have wasted time we could have spent looking at other facilities. It was rude, and entirely unnecessary.
