Dr Hale and his staff have taken over Three weeks to fill out paperwork required for short term disability. Upon dropping off the paperwork I was told it would take seven days to fill out. This was incorrect. First time I dropped the paperwork off, the front desk lost it. It had personal information on it. I’m sure that is a huge HHIPA violation. This second round of paperwork has now been over Three weeks. I have emailed and called and can’t get the assistant to call me back. Dr. Hale came highly recommended for my surgery and everything is set up. My HR dept is on me to have my paperwork in 30 days prior to surgery. That date is almost upon me and havnt heard anything. I am very disappointed and hope that my surgery goes better.
