I’ve been trying to request records on a patient of Dr. Davis’ for weeks(with a signed authorization from the patient) but keep running into obstacles. At this point it feels intentional and like they do not want us to have the records. We originally requested the records on 10⁄6 and they were faxed over the following day– no problem. So naturally when we requested the records again on 11/30, we assumed the process would be the same but there was a hold up; first, «the request was sitting on the doctor’s desk but he hadn’t approved it yet,» then they would not accept our request because it had the birth mother’s maiden name instead of her married name(even though the first time we requested her records, we requested them using her maiden name and they released them to us). So, we sent over a second request on 12⁄7. Then, on 12⁄11 they sent a letter telling us that they were going to charge us $ 25 to fax 16 pages of records, a fee they failed to mention the first time we requested records and they sent them over without any charge. We mailed the check on Monday 12/14, the day we received their letter requesting the $ 25 fee. We called two days later to confirm their receipt of the check. They said they received it and would fax the records to us that day(yesterday). When our administrative assistant came in this morning and saw that they never faxed the records, she called this morning and they did not answer and then she called back five minutes later and it rang twice and then went to voicemail(both times she called were after 9:00 am). They faxed the records 5 minutes later. Very difficult to work with.
